How to Enroll
Welcome to our school!
We are happy for you join our school family. Below you will find the list of required documents necessary to enroll within Orange County Public Schools. Once you have gathered all the necessary documents and completed the registration packet, you will be able to register here at the school.
To register your student in school, the following documentation is necessary:
Verification of Legal Name
• Birth Certificate
Verification of Age* (with one of the following):
• Birth Certificate or Passport
To enter Kindergarten, a child must be 5 years old on or before September 1.
To enter first grade, a child must be 6 years old on or before Sept. 1 and have completed Kindergarten.
Verification of Immunization and Physical Exam
• Proof of immunizations on a Form 680, which can be obtained at the Orange County Health Department;

• Proof of physical examination by a U.S. doctor within a year of enrollment (first day of entry at school).
Verification of Academic History
• Transcript, Withdrawal Form, Last Report Card
Verification of Special education information (if applicable)
• Current IEP or Current 504 plan
Verification of your residence in Orange County (with one of the following):
• Home ownership: Current Homestead Exemption Card, current property tax statement, closing contract,
or warranty deed, along with current utility bill, ID with current address, and current mail.
• Renting/leasing: Current signed lease, ID with current address, current utility bill, rent receipts, current
mail, and renter’s insurance.
• Living with someone who owns or rents the home: Verification of Residence from Student Enrollment
(only by appointment: https://appointments.ocps.net)

More Documentation may be required depending on circumstances in a case-by-case situation.
Verification of Guardianship
• Birth Certificate
If applicable, you must provide one of the following:
• Court Documentation (such as divorce decrees with a w/parenting plan or the placement of children through the court)
• OCPS Educational Guardianship (given only when the parent/guardian lives outside of Orange County or adjacent
counties of Brevard, Osceola, Polk, Lake, Seminole, and Volusia) available at:

*Other forms of age verification are permissible under Section 1003.21, Florida Statutes
Temporary Documentation Exemption: Students who lack a fixed, regular, and adequate nighttime residence have a right
to immediate enrollment under the McKinney-Vento Homeless Assistance Act, 42 U.S.C. 11435. A completed Student
residency Questionnaire is needed to determine eligibility (pages 7-8). The student residency questionnaire is two pages.

• Certificado de examen físico por un médico de los EE. UU. dentro del año de inscripción (primer día de ingreso a la escuela).
Acreditación de historial académico
• Expediente académico, Formulario de retiro, Ultima libreta de calificaciones
Acreditación de información de educación especial (si corresponde)
• IEP (Programa de educación individualizado) actual o Plan 504 actual
Acreditación de residencia en el condado de Orange (por medio de una de las
siguientes opciones):
Propiedad de vivienda: Tarjeta de exención de vivienda actual, declaración de impuestos a la propiedad actual,
contrato de cierre o escritura de garantía junto con factura de servicios públicos actual, identificación con dirección
actual y correo actual.
Alquiler/arrendamiento: contrato de arrendamiento firmado vigente, identificación con dirección actual, factura
de servicios públicos actual, recibos de alquiler, correo actual y seguro para inquilinos.
Convivir con alguien que sea propietario o alquile la vivienda: Verificación de Residencia a partir de
Matrícula de Estudiantes (solo con cita previa: https://appointments.ocps.net)

Please pay special attention to the requirements for Verification of Residence and Verification of Guardianship. For some families, it may be necessary to visit the Office of Student Enrollment to receive the proper documentation BEFORE registering here at the school.
Required Documents

Official Birth Certificate
- A duly attested transcript of the child’s birth record filed according to law with a public officer charged with the duty of recording births.
OR
Passport
- A passport or certificate of arrival in the U.S. showing the name of the child.
NOTE: Under no circumstances shall staff request a passport, visa, or any other documentation to verify the immigration status of any student.

*Students may attend school without a Florida Certificate of Immunization if they have a:
- Religious Exemption (Form DH 681)
- Temporary Exemption (Form DH 680, Part B)
- Medical Exemption (Form DH680, Part C)

Enrollment takes place at the school with the school registrar if you have the following as proof of residence, at a minimum:
- Own Home/Mortgage: Provide the current homestead exemption card, current property tax statement, closing contract, or warranty deed along with current utility bill, ID with current address, and current mail as your proof of residence.
- Rent/Lease: Provide the current signed lease, ID with current address, current utility bill, rent payments, current mail, and renters insurance as your proof of residence.
The following situations require a Verification of Residence obtained from the Office of Student Enrollment BEFORE enrolling at the school.
- Living with someone who owns or rents the home:
If you don’t have a lease or own your own home (share housing with someone else) you must obtain a Verification of Residence from Student Enrollment as your proof of residence by providing a minimum of the following documents:
- Parent or Guardian's Florida driver's license or state ID showing the current correct address.
- Another three (3) items in the Parent or Guardian's name showing the current address, such as: auto registration, current mail, bank statement, insurance, phone bill, or current pay stub.
- Homeowner or Lessee's driver's license or state ID, homestead exemption card, property tax information, or signed lease agreement.
- More documentation may be required depending on circumstances in a case-by-case situation.
- Own Home/Mortgage: Provide the current homestead exemption card, current property tax statement, closing contract, or warranty deed along with current utility bill, ID with current address, and current mail as your proof of residence.

- Court Custody Documentation (this includes divorce decrees)
- OCPS Educational Guardianship: This documentation is provided by the Office of Student Enrollment when the parent/guardian lives outside of Orange County or adjacent counties of Brevard, Osceola, Polk, Lake, Seminole, and Volusia).
